We look forward to working with you to create your online profile for My Choice Adoptions. Follow the simple steps below to begin!

We'll be using a shared folder on Dropbox so you can easily upload your photos and completed intake form.

Step 1

Please send us an email at adopt@ourchosenchild.com to let us know you're working with My Choice Adoptions and ready to begin your online profile and we'll get your Dropbox folder set up.

Please review, download, and carefully complete the Online Profile Intake Form.

Step 3

Please upload your completed Online Profile Intake Form, photos, and adoption profile PDF to your shared Dropbox folder.

Step 4

Once you have finished and uploaded your form, content, and photos to our shared Dropbox folder, please email adopt@ourchosenchild.com to let us know that your content is ready for us to review!

Ready to begin?

Once your content is approved, we will design your online profile and have it ready within 2 business days. You will have an opportunity to review your online profile and suggest any changes you'd like us to make at that time.
