Set Up Your Adoption Video
expectant parents are online and you should be too!
Your online presence gives expectant parents a glimpse into your life. The following form will take you through the process of compiling the information we need to set up your adoption video for The Adoption Alliance. We are always available to answer any questions you have—just shoot us an email or call us and we’ll be in touch shortly.
here are a few notes to make submitting your files as easy as possible!
Step One
Saving your progress to continue later is easy! Click the save icon at the bottom of the form when you're ready to take a break.
Step Two
Once you click the save icon it will expand to say "Save & Continue Later"
Click "Save & Continue Later"
Step Three
Enter the e-mail address you would like your partially completed form to send to. You MUST send your partially completed form via email to continue filling it out later.
Step Four
When you're ready to continue filling out your form, find the e-mail (from JotForm) and follow the link back to your form.
If you don't see the e-mail in your inbox be sure to check out your spam folder.

what you’ll need to get started on your adoption video
I will use your completed adoption profile to create your adoption video. That means that you don't have to worry about uploading new photos or writing new text (yay!).
All I need from you are 5-15 short video clips to add to your video (The Adoption Alliance requires that you have at least 5 clips).
What type of videos should you upload? Short clips where you're not speaking directly to the camera about your adoption. These video's don't need to be anything fancy, they can be saved from your cell phone! Here are a few ideas:
Video of you with your child or children (if applicable)
Video playing outside, in the park, or in the snow
Video of a birthday celebration
Video of a Christmas celebration
Video playing with your pets
Video at a concert, ballgame or some type of activity
Don't include video's of you speaking directly to the camera about adoption
Do you have additional photos you'd like to include that aren't in your adoption profile book? There is a section at the end of the form where you can upload new or alternate photos.
